Program Coordinator β MDH at Home
0.8 EFT (64 hours per fortnight)

πΏ About the Role
Mansfield District Hospital is seeking an organised and community-focused Program Coordinator to lead the coordination of our MDH at Home program.
MDH at Home delivers government-funded aged care services directly to older members of our community in the comfort and safety of their own homes. This vital program supports independence, dignity and quality of life for elderly clients while ensuring access to essential care services.
In this key coordination role, you will manage program intake, oversee waitlists, and coordinate funding arrangements in collaboration with external service providers. You will act as a central point of contact, ensuring clients experience a seamless and responsive service from referral through to service delivery.
π Key Responsibilities
- Coordinate program intake, assessments and client onboarding
- Manage and monitor waitlists to ensure timely service access
- Liaise with external providers regarding funding arrangements and service delivery
- Maintain accurate documentation and reporting requirements
- Support the Nurse Unit Manager β Community Care with program oversight and administration
- Ensure compliance with relevant government funding guidelines and aged care standards

You are highly organised, detail-oriented and confident working across multiple stakeholders. You understand the importance of community-based aged care and are passionate about supporting older people to remain living safely at home.
You bring:
- Experience in health, community services or aged care coordination
- Strong administrative and organisational skills
- Excellent communication and stakeholder engagement abilities
- Knowledge of government-funded aged care programs (desirable)
- Ability to manage competing priorities and work independently
