Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.

The primary role of the Community Care Intake Advisor (CCIA) is to support timely response to and management of referrals to NHW community clinics and services.

The CCIA forms an integral part of the Community Care Intake team which forms the central point for service access to NHW community clinics and services and is responsible for applying eligibility and intake guidelines to respond to referrals and, where delegated, to apply a clinical prioritisation framework that supports timely clinical triage.

The CCIA role requires a high level of organisation skills and attention to detail, as well as exceptional communication skills and experience working closely with medical, nursing and allied health clinicians.
