The Department

Caulfield Community Health (CCH) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. CCHS operates from the Caulfield Hospital site at Bayside Health.

The Role

The Dietitian will work within the multidisciplinary Adult Health at Caulfield Community Health (CCH), where intervention can be provided in the centre, community and client’s homes.

The Dietitian will provide comprehensive nutrition assessment and management for CCH clients including those with chronic health conditions, swallowing or chewing difficulties, malnourished, overweight or obese, gastrointestinal disorders as well as general dietary advise. The majority of the clients will be over 65 and referred via My Aged Care under the Commonwealth Home Support Program.  CCH operates from the Caulfield Hospital site at Bayside Health.

Key Duties and Responsibilities

  • Provide high quality nutrition services to support clients who live in the CCH catchment.
  • Provide comprehensive and timely nutrition and dietetic assessment, treatment, education, referral and discharge of clients in line with professional standards and organisational guidelines.
  • Develop and implement person centred care plans.
  • Liaise with relevant agencies to coordinate, facilitate and communicate client treatment and outcomes.
  • Demonstrate an understanding of pathways within the health care system in order to help facilitate appropriate care and advocate for clients with complex care needs (e.g. My Aged Care and Commonwealth Home Support Program).
  • Maintain a culturally sensitive approach to the planning, delivery and evaluation of services and programs to ensure they are appropriate and relevant to all culturally and linguistically diverse (CALD) groups in the local community.
  • Facilitate referrals and other client related admin tasks as required.
  • Plan and manage own workload to ensure adequate time for the achievement of primary objectives of the position.
  • Enter accurate service provision data for reporting as required.

Qualifications and Experience Required

Essential 

  • Tertiary qualifications in Nutrition and Dietetics or equivalent
  • Eligibility to be a Certified Accredited Practicing Dietitian
  • Eligibility for DAA Membership
  • Demonstrated dietetic clinical experience (minimum 3 years) relevant to the community health sector which could include experience and skills in a range of dietetic specialities including chronic disease and aged care
  • Capacity to work independently and autonomously and yet facilitate a collaborative culture
  • Experience in working as part of a multidisciplinary team to provide comprehensive and holistic services
  • Understanding of, and alignment with, the Bayside Health core attributes
  • Excellent communication, collaboration and problem-solving skills
  • Well-developed computer skills
  • Current Victorian driver’s licence

Desirable

  • Experience working within the general medical, aged care, or community health sector
  • Experience in supervision of students
