About the Team  

The Caulfield Hospital team specialises in community-based care, rehabilitation and aged care services. Our services extend across hospital, community and home-based settings, allowing care to be tailored to individual patient needs.   

Our pharmacists are recognised as essential members of all clinical teams, actively engaging in a unit and team-based care model involving ward rounds and collaborating closely with the multidisciplinary team to achieve excellent patient care outcomes.  

The service has 3 key priorities:  

  • Practice
  • Education
  • Research  

The department also has advanced Education programs with Foundation Residency and Advanced Training Residency to support career progression, including opportunities to rotate to external hospitals. 

About the Role  

This is an exciting opportunity for an experienced clinical pharmacist to take on a leadership role in Alfred Health’s Health of Older People Program which provides care to patients across a continuum of inpatient, home based and community settings. It specialises in care for people with chronic or complex conditions associated with ageing (e.g. dementia, falls, continence and bone health), memory and thinking problems, chronic illness or disability. 

As Senior Clinical Pharmacist – Health of Older People, you will lead the development and delivery of pharmacy services within HOP inpatient settings, contribute to service improvement and evaluation initiatives, and act as a mentor for junior staff and students. You will also play a key role in advancing safe and effective use of medicines in the community setting. 

Key Skills  

  • Ability to demonstrate leadership and decision making skills
  • Problem-solving and risk assessment abilities and the ability to use initiative
  • Excellent communication, time management skills with the ability to prioritise, plan and organise your work and deliver to deadlines  
  • Ability to work effectively in multidisciplinary teams  

Qualifications & Experience required  

  • Registered pharmacist in Australia
  • Relevant post-graduate qualification in clinical pharmacy or leadership and management highly desirable
  • Extensive hospital pharmacy experience, with a demonstrated understanding of contemporary practice in hospital pharmacy services highly desirable
  • Experience in leading and coaching highly effective team (formally or informally) desirable
