The Department

Health of Older People provides specialised subacute care for older adults requiring further medical assessment, rehabilitation, and support with complex discharge planning. Our services are designed to promote independence, dignity, and quality of life for patients in later stages of care.

We are currently seeking a committed and experienced Clinical Support and Development Nurse to join our team on Ward ACG—a dynamic unit delivering high-quality, patient-centred care to older adults with complex needs.

Ward ACG

Ward ACG is a 30-bed unit comprised of:

  • A 15-bed Geriatric Evaluation and Management (GEM) service, focused on subacute rehabilitation for older adults following acute hospitalisation
  • A 15-bed secure Delirium and Dementia unit, dedicated to supporting patients experiencing acute cognitive impairment, including behavioural and psychological symptoms of dementia (BPSD) or delirium

Patients on Ward ACG often present with complex medical, functional, cognitive, and psychosocial needs. The unit provides a supportive and collaborative care environment, where staff work closely within a multidisciplinary team to develop and deliver personalised care plans aimed at maximising recovery and quality of life.

The Role

The Clinical Support and Development Nurse plays a key leadership role in supporting the growth and capability of the nursing workforce and improving the quality of care provision on the ward. Working alongside the Nurse Unit Manager and supported by the Nursing Education team for the Program, this position focuses on:

  • Supporting quality improvement initiatives and contributing to the creation of a culture of learning and continuous improvement
  • Monitoring the clinical performance of the ward in consultation with the Nurse Unit Manager and supporting the team to develop initiatives for required practice change and improvements
  • Leading the development of preceptors and supporting the orientation of new team members and graduate nurses
  • Identifying the professional development needs of the nursing team and designing learning and career pathways in collaboration with the Nurse Unit Manager and Nursing Education team
  • Promoting evidence-based practice and clinical excellence through education and role modelling
  • Providing mentorship, clinical guidance, and day-to-day support to nursing staff

About You

We are looking for a Registered Nurse who brings:

  • Clinical experience in subacute, geriatric, or dementia care nursing
  • A passion for clinical leadership, quality improvement, education and mentoring
  • Strong communication and interpersonal skills
  • A commitment to team-based, person-centred care
  • An ability to promote reflective practice and continuous improvement
