The Department

The Hepatitis C Integrated Services Clinical Nurse Consultant (CNC) will work closely with both the Department of Infectious Diseases and the Department of Gastroenterology medical specialists to provide and expanding access to Hepatitis C treatment.   The Department Infectious Diseases will identify internal clinical programs and external health services, to partner with to implement and sustain, targeted, systematic Hepatitis C screening/testing and develop pathways/linkages to community based treatment and follow up.

The Role

The CNC will be central in facilitating and implementing this clinical response with the targeted clinical programs and services.  Working closely with the Head of the Hepatitis C Service, Department of Infectious Diseases the CNC will develop strategies with the clinical programs to enhance and sustain screening/testing.  The CNC will ensure appropriate assessment and a treatment plan is in place prior to hospital discharge and facilitate linkage to treatment and/or follow up in the community.   Within the community settings and services the CNC will support GPs and nurses to screen/test and develop treatment and follow up plans along with providing direct service provision.  They will also be a key liaison point between community based services and specialist services.   The role will involve collaborating with inpatient and outpatient clinical services across Alfred Health.  The CNC will develop, support and ensure data systems are in place to monitor and evaluate the response/program in each clinical program. 

The CNC will develop strong relationship with community based treatment services to ensure effective pathways from the hospital setting to community setting for Hepatitis C treatment and follow up.  The response will vary in each setting with some being a direct nursing role assessing, screening, testing and facilitating treatment access.  In other settings the role will focus on supporting local clinical staff work directly with the patient with the CNC role that of clinical support, monitoring, linkage to Hepatitis C specialists where required and ensuring an appropriate plan is in place to link the patient to Hepatitis C treatment and monitoring if required.  The role will also be responsible for obtaining treatment outcome data for patients discharged to community care.   

The CNC will also provide a fibroscan clinic in Infectious Diseases Outpatients and provide inpatient fibroscan assessments where required.  The CNC will be a resource nurse for the Department of Infectious Diseases Hepatitis Outpatient Clinic.  The will also oversee the maintenance of a liver cirrhosis clinical review database for patients of the Department of Infectious Diseases and follow up patients who require review.  The role will provide Hepatitis C fibroscan assessment clinics, linkage to specialists and targeted screening to targeted primary health care clinics. 

The role will evolve with changes in Hepatitis C testing and treatment and with some clinical sites reaching greater levels of independence managing Hepatitis C.  The CNC will be expected to lead new initiatives and projects to assist the evolution of the role and identify new responses to support the goal of eliminating Hepatitis C. 

QUALIFICATIONS/EXPERIENCE REQUIRED

  • Essential
    • Current registration with the AHPRA
    • Relevant experience working with people infected and affected by Hepatitis C virus or related field
    • Post graduate qualification in a relevant area is desirable
