The Department

General Medicine Hospital in the Home (Gen Med HITH) is looking for nurse clinicians to join the Gen Med HITH team. This is an exciting opportunity to expand your knowledge, and experience caring for complex patients with acute conditions in the community setting. The service cares for patients that would normally be cared for in a hospital bed. Instead they receive their care in the comfort of their home, with the support of nurses, allied health, and medical team members. The nurse clinician is a key role in the multidisciplinary team, ensuring that patient care is holistic and that patients have robust follow up plans following their acute therapy. Gen Med HITH works closely with the acute inpatient wards and the emergency departments, and collaborates closely with the Hospital Admission Risk Program (HARP) and other community-based programs.

The Role

The Gen Med HITH Nurse Clinician role offers an excellent opportunity to stretch your learning about community-based acute care while working in a friendly and supportive team. Successful applicants will have a Victorian driver's licence, and recent acute clinical care experience. Cannulation skills, and a strong knowledge base in chronic disease management including heart failure, diabetes, and wound management are highly advantageous. Excellent problem solving, collaboration, the capacity to work autonomously within role scope, and a can-do attitude are essential.

Experience and Qualifications required

  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • Experience within the acute and or ambulatory/community setting desirable
  • Current Victorian Driver's Licence
  • Recent acute clinical care experience
  • Strong knowledge base in chronic disease management including heart failure, diabetes, and wound management are highly advantageous
  • Excellent problem solving, collaboration, the capacity to work autonomously within role scope, and a can-do attitude are essential
