The Department

The Hospital Admissions Risk Program (HARP) is a Health Independence Program (HIP) and sits within Alfred Health’s Home, Acute and Community (HAC) Program. As defined by the Department of Health and Human Services (DHHS), HARP aims to “provide hospital substitution and diversion by supporting people in the community, in ambulatory settings, and in people’s homes.”

Alfred Health’s HARP service is delivered by a multidisciplinary workforce operating within an interdisciplinary model of care.

We are currently seeking an experienced Social Worker with a strong interest in chronic and complex holistic health care. The ideal candidate will have demonstrated experience in coordinating and supporting individuals with complex psychosocial needs, as well as leading and mentoring clinicians within a multidisciplinary team.

The Position

The HARP Senior Clinician - Social Work plays a key role in providing clinical leadership and coordinating service delivery within the centralised HARP Allied Health team. The Senior Clinician - Social work provides professional leadership/supervision of the Case Management/Social Worker unit (currently 4 Social Workers), and contributes to operations and activity of that unit. As a leader within the HARP AHP cohort, they may be expected to support physiotherapists, occupational therapists, and dietitians. The Senior Social worker will also hold a small caseload of their own, in-line with their EFT.

Key Responsibilities

  • Providing secondary consultation and clinical support across the HARP team
  • Leading initiatives to enhance clinical and professional skills, including student development
  • Organising and facilitating case conferences to support complex discharge planning from inpatient wards, the Emergency Department, and HARP
  • Collaborating closely with the HARP Manager, operational leads, and clinical staff
  • Delivering discipline-specific supervision and professional development, including support for clinicians from non-social work backgrounds
  • Coordinating education sessions to ensure staff remain current with contemporary psychosocial practices and foster a culture of continuous learning
  • Managing a caseload of clients, providing expert assessment and care planning
  • Contributing to service improvement and research activities within HARP and Alfred Health
  • Representing the service to support state-wide psychosocial outcomes for patients

Qualifications and Experience Required

  • Hold a Degree or equivalent qualification
  • Be eligible for membership of the Australian Association of Social Workers
  • Meet AASW standards
  • Comply with the Code of Conduct as per Schedule 2 of the Health Complaints Act 2016
  • Agreeable to provide an annual Statutory Declaration stating that credentialing requirements have been met for Social Workers at Alfred Health
  • Extensive clinical experience in and knowledge of patient-centred, best practice
  • Extensive clinical experience in relevant community sectors. i.e. Community case management or management of complex chronic diseases
