About the Ward

The ACG Ward at Caulfield Hospital is a specialized, secure 15-bed unit dedicated to the care of patients experiencing subacute conditions, delirium, and/or dementia. Our multi-disciplinary team provides slow-stream rehabilitation, focusing on managing complex cognitive behaviors and supporting patients to achieve their optimal outcomes for a safe discharge home.

The Role

Are you passionate about enhancing the quality of life for older adults? We are seeking a compassionate, energetic Lifestyle Coordinator to join our team.

In this highly rewarding role, you will work under the direction and supervision of a Registered Nurse to deliver meaningful therapeutic, lifestyle, and diversional activities. Your primary goal will be to provide a calm, reassuring environment that reduces patient agitation, improves mood, and helps individuals with cognitive impairments maintain dignity and function.

Key responsibilities include:

  • Therapeutic Programs: Designing and delivering daily exercise programs to improve patient mobility, alongside music therapy, card games, reading, and socialisation activities.
  • Behavioral & Environmental Support: Helping maintain a safe, tranquil, and tidy ward environment that minimizes the risk of falls and supports confused or disorientated patients.
  • Nutritional Care: Assisting with the setup of communal dining and providing patient mealtime/feeding support where clinically appropriate.
  • Team Collaboration: Communicating effectively with patients, families, and the broader interdisciplinary team to ensure patient-centred care goals are achieved.
  • Safety & Observation: Promptly identifying and reporting changes in a patient's physical or mental condition to the supervising Registered Nurse.

About You

To be successful in this role, you will bring a warm, patient-centred approach and a genuine empathy for patients experiencing cognitive decline. You are an excellent communicator who excels in a collaborative team environment and knows how to build immediate rapport with patients and families.

Qualifications and Experience Required

Essential

  • Certificate III in Health Services Assistance (Acute), or a willingness to complete additional units.
  • Good computer skills and a strong commitment to a professional work ethic and ongoing learning.

Desirable

  • Prior experience in an acute or subacute healthcare setting, particularly working within aged care, dementia care, or rehabilitation environments.
  • Experience utilizing modern hospital information or electronic health systems.
