The Department

The Home, Acute and Community (HAC) Program at Alfred Health plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, while minimising long-term care needs. 

The Older Person’s Care Hub (OPCH) is a new, grant-funded pilot program within the HAC portfolio. The OPCH aims to enhance integrated care for older adults through multidisciplinary, home-based assessment and intervention, supporting older people to remain safely in their homes and reducing avoidable hospital presentations and admissions.

The OPCH will be based alongside the Mobile Assessment and Treatment Service (MATS) at Caulfield Hospital and will work closely with GPs and EDs for referrals in the first instance. This initiative will add to the growing suite of home-based services provide by the HAC Program ensuring coordinated transitions of care and continuity of support. The pilot will run for an initial 12–18-month period, with patient care commencing from January 2026.   

The Role

We are seeking an experienced and dynamic Registered Nurse to join Alfred Health’s Older Person Care Hub (OPCH) pilot – an innovative model of care designed to reduce hospital presentations and admissions for older people.

As the OPCH Nurse, you will provide clinical care and coordination within a multidisciplinary team, delivering person-centered care in home, telehealth, and clinic settings. You will work closely with the OPCH Team Leader, Geriatrician, and allied health professionals to optimise health outcomes, support older people to remain at home, and contribute to service development and evaluation.

This is an exciting opportunity to shape a new model of care and make a meaningful impact on the lives of older people through integrated, community-based service delivery. 

Qualifications and Experience Required

Essential

  • Current registration with AHPRA as a Registered Nurse
  • Minimum 5 years clinical experience, preferably in acute, general medical, or geriatric nursing
  • Sound clinical knowledge of complex health and medical conditions
  • High level of computer literacy and competence in patient management systems.
  • Ability to work positively and flexibly with internal and external services
  • Excellent written and verbal communication skills
  • Demonstrated interpersonal skills and ability to work collaboratively
  • Commitment to continuing professional development
  • Current Victorian Driver’s Licence

Desirable

  • Postgraduate qualifications in gerontology, community health, or palliative care nursing
  • Recent experience in general medicine, aged care, emergency, or critical care nursing
