The Department

HARP (Hospital Admission Risk Program) Complex Care is a community-based service within the range of Health Independence Programs (HIP).  Alfred Health HARP sits within the Alfred Home Acute and Community (HAC) Program and reports through the Acute and  Integrated Stream (AIC). HARP provides education and care coordination to improve the health literacy, well-being, and health independence of individuals living in the community.

The Role

This position is open to experienced registered nurses with a strong interest in chronic and complex holistic health care. This role works within a geographic team with supervision and support from the  Complex Care Coordinators, working alongside the Alfred case managers, social workers, and allied health clinicians. The HARP RN is responsible for conducting phone risk assessments, triaging new referrals, and prioritising care. During outreach visits, the HARP RN conducts comprehensive and holistic systems assessments, initiates interventions  to improve patients' health independence, and provides support and education on the self-management of chronic disease. The HARP RN works in collaboration with the patient, family, GPs, the HARP medical registrars, carers and other community based services or hospital based resources. The RN works in partnership with the multidisciplinary team and the HARP registrar in the holistic care of patients.

Qualifications and Experience Required

Essential

  • Four or more years of nursing experience following graduating from a Bachelor of Nursing
  • Sound assessment skills, critical thinking and the ability to work autonomously within role scope and as part of a team.
  • Skills in improving patients' health states and quality of life through the promotion of chronic disease self-management and health independence   
  • Ability to rapidly recognise deterioration and or risk in the community and to escalate to the appropriate clinical and managerial line of management
  • A strong interest in social equity and in caring for people who experience significant challenges in both chronic disease, vulnerability, and psychosocial complexities
