As a key member of our Community Health team, you will deliver safe, high‑quality physiotherapy services to residents, patients and community clients across residential aged care and outpatient settings. You will provide clinical assessment, treatment planning, intervention and consultation, working collaboratively with our multidisciplinary team to ensure continuity of care.
This role has no direct reports, but includes supervisory responsibilities for Allied Health Assistants and graduates, including guiding and setting treatment plans.
Key Responsibilities
- Provide consumer‑focused assessment, treatment and consultation within scope of practice
- Work collaboratively with the multidisciplinary team to support smooth transitions of care
- Develop and document clear, person‑centred goals and treatment plans
- Deliver evidence‑based physiotherapy interventions and adjust plans as required
- Maintain accurate, timely and legally compliant clinical documentation
- Ensure safe and appropriate use of equipment and report maintenance needs
- Delegate suitable tasks to Allied Health Assistants and Graduates to support their practice
- Contribute to data collection, reporting and quality improvement activities
- Communicate respectfully and professionally, modelling YDMH values
