After Hours Coordinator (AHC)

Permanent Part-Time

32 hours per week (0.8EFT)

The After Hours Coordinator (AHC) is part of a team of senior nurses responsible for the after hour management and coordination of services at Yarrawonga Health. This includes the delivery of safe, competent and responsible clinical care to all clients of Yarrawonga Health.

The After Hours Coordinator is responsible for the coordination of all hospital resources, admission of patients, compliance to the Safe Patient Care Ratio legislation and may be required to assist in the provision of direct nursing care in any department within the hospital.

Qualifications:         

  • Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and hold a current practicing certificate.
  • Tertiary qualifications in management or working towards same highly desirable

Key Selection Criteria:

Essential:

  • Current registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Minimum Five (5) Years’ Clinical experience in Advanced Clinical Practice
  • Residential Aged Care knowledge and experience
  • Ability to operate effectively in an environment of change and continuous improvement
  • Demonstrated ability to manage resources effectively
  • Demonstrated leadership skills and ability to function independently
  • High level effective communication skills
  • Ability to work effectively within a multi-disciplinary team
  • Experience with clinical performance monitoring, budgeting and resource management
  • Proven knowledge of information management systems
  • Demonstrated understanding of contemporary health service issues

Desirable:

  • Membership of relevant professional body
  • Tertiary qualifications in management or working towards same highly desired
  • Knowledge of current issues, trends and research in Aged Care

Key Responsibilities:

Accountabilities:

  • Promote an environment where Registered Nurses and Enrolled Nurses practice in accordance with the Nursing and Midwifery Board of Australia – Nursing standards of practice for the Registered Nurse and Enrolled Nurse;
  • Provide appropriate interventions and escalation of events impacting staff to ensure that appropriate support is provided for nursing staff with identified professional or personal needs, setting and evaluating objectives where required, in conjunction with relevant personnel;
  • Provide leadership relevant to the clinical focus of this role in line with meeting Yarrawonga Health strategic objectives;
  • The After Hours Coordinator is required to coordinate staffing requirements for all areas within Yarrawonga Health. These will include the recall of appropriate staff for all medical, surgical, diagnostic procedures and facility problems or concerns;
  • The After Hours Coordinator is accountable for the standards of patient care and nursing practice undertaken out of hours. They are also required to provide information pertaining to issues that are specific to the after-hours management of the organisation;
  • The After Hours Coordinator will demonstrate commitment to person-centred care, best practice standards.

Clinical services:

  • Provide leadership and direction within Clinical Services for clinical care practice standards. Facilitating the delivery of a high standard of evidence-based nursing care throughout the organisation;
  • Maintain accurate and current clinical records ensuring documentation meets professional and legal standards;
  • Promptly report and escalate vital patient observations that are outside the normal range as per clinical guidelines;
  • Ensure all clinical practice is delivered within legislative guidelines.

Provide quality and safe clinical care for consumers:

  • Ensure patients, families/representatives or carers are provided with timely information and are involved in decision making about their care planning and treatment;
  • Accept accountability for own actions and seek guidance when a situation exceeds experience and/or knowledge;
  • Uphold and protect patient’s rights, maintain strict confidentiality and continually practice to the principles of open disclosure;
  • Consumer feedback and complaints are responded to appropriately to ensure that issues are resolved and quality and safe clinical care is provided.
