Social Worker Grade 1 or 2

About the Role:

As a key member of our multidisciplinary team, you will provide high-quality, person-centred support across acute, subacute, residential aged care, and community programs. You’ll play a vital role in:

• Providing psychosocial assessments and interventions
• Supporting patients through transitions of care and discharge planning
• Advocating for client needs and linking with community services
• Working closely with patients, families, carers, and healthcare teams

What We’re Looking For:

• Demonstrated experience in a healthcare or community setting
• Strong communication, advocacy, and interpersonal skills
• A commitment to collaborative, culturally sensitive, and trauma-informed care
