Role overview:
- Support children, young people, families, and carers as they engage with the Stride Kids service, helping create a welcoming, connected, and family-inclusive experience.
- Work alongside families to support engagement with the service, assist with navigating referral pathways and community supports, and reduce barriers to accessing care.
- Provide practical, emotional, and systems-based support to families and carers, including advocacy, service coordination, and linking with relevant supports where appropriate.
- Support intake, engagement, and transition processes for children, young people, and families entering or exiting the service.
- Develop and maintain collaborative relationships with clinicians, schools, community organisations, and other stakeholders to support coordinated care.
- Attend and contribute to community and interagency meetings on behalf of the service to strengthen partnerships and referral pathways.
- Respectfully and effectively respond to issues raised by clients, carers, family members, and community stakeholders.
- Maintain accurate documentation and contribute to a positive, safe, and supportive team environment.
Requirements:
- Minimum of a Diploma in mental health or community services related discipline (ideally a completed Degree)
- Previous experience in a Support Worker, or related, role
- In- depth understanding of issues pertaining to mental illness/or disabilities and their impact on individuals, families and communities.
- Background knowledge and understanding of drugs, alcohol and illicit drug use impacting upon the health of individuals and families
- Experience in group facilitation
- The ability to work autonomously, under supervision and as part of a team.
- Demonstrated effective listening, empathy and communication skills
- Excellent negotiation, problem solving and decision making skills, as well as the ability to work to time frames, meet deadlines, and work in challenging situations.
- Demonstrated ability to work with families, carers, government agencies and community services
- Willingness to abide by the Disability Services Acts and Standards and Work Health & Safety Act
- Competence with Microsoft Office
- Must be eligible to apply/currently hold a Blue Card
- Must hold/be willing to apply for NDIS worker screening clearance
- Current Driver's license
