Role overview:

  • Providing high quality evidenced based clinical care as a member of the multidisciplinary team
  • Conduct comprehensive biopsychosocial assessment, coordinate early intervention mental health care, and provide specific psychological and psychosocial interventions for a caseload of young people within a clinical case management model
  • Address barriers and challenges to accessing appropriate service pathways and effective treatment through the provision of enhanced access, inclusive of out of office hours, outreach, home based care and telehealth services
  • Facilitate a culture of family involvement and offer - family based interventions where appropriate
  • This is a key position within internal clinical escalation pathways, and is responsible for overseeing the intake and assessment process of new young people to Uplift. May be responsible for contributing to/and or leading a range of service/quality improvement activities within Uplift or across headspace by Stride services
  • You will be required to work one evening per week (Tues or Wed 11:00 - 19:30)
  • May be required to work from a number of other sites in the region and in an outreach capacity to meet young person and service requirements, as well as afterhours work

Requirements:

  • Must have one of the following: Gereral AHPRA registration as a Psychologist/Clinical Psychologist Mental Health Occupational Therapist, Mental Health Nurse OR AASW Accredited Mental Health (or working towards) Social Worker
  • Knowledge and skills which which have been gained through qualifications and previous experience in a mental health service delivery environment (preferably 3 years minimum post graduate experience)
  • Ideally skills and knowledge with respect to the assertive engagement, case management, crisis intervention and management of young people presenting with complex needs
  • Ability to participate in a multidisciplinary team utilising high level communication skills to establish effective partnerships, and provide consultation to the team as well as government and other non-government service providers.
  • Demonstrated level of knowledge and skill to autonomously undertake routine clinical practice, inclusive of assessment and care planning, with a sound understanding within the continuum of care
  • Must be eligible to apply/currently hold a Blue Card
  • Must hold/be willing to apply for NDIS worker screening clearance 
  • Must have a drivers license
