Opportunity Pathways Program facilitator

The OPP aims to build the skills and knowledge for social and economic well-being and increased economic participation of clients. It delivers specialised employment services to Clients through a tailored, needs-based case management approach. The relationship with the Case Manager is the foundation on which the OPP case management process is based, working in partnership with Clients and other service provider personnel and professionals involved in supporting the Client.


To provide holistic, culturally appropriate, person-centred case management service under Opportunity Pathways Program (OPP) to social housing tenants (Clients), including those with complex needs requiring specialised and intensive services, to build the skills and knowledge for social and economic well-being and participation. The Case Manager will be the central point of contact for Client referrals.

Your main duties would include:

  • Deliver Opportunity Pathways Program to provide, manage, coordinate and facilitate Immediate Deliverables, (including Education and Employment outcomes), and orientation components of OPP to Client through person-centred and strengths-based case management
  • Conduct Needs Assessment and ensure the needs assessment adequately and accurately captures the support needs of individual clients
  • Implement CMP via service delivery methods including working in partnership with the Client, other SSI staff and programs (e.g. support workers, housing officers, bilingual guides, volunteers, and other staff from RESP, Ignite, Ability Links, etc programs) and other Service Provider personnel (such as jobactive, DES, AMEP provider(s), NSW Health Services, mental health specialists, substance abuse counsellors, GPs) and professionals to achieve OPP Outcomes
  • Maintain regular contact with clients to (i) track Client progress against the CMP, (ii) record achievements and/or completion, (iii) reassess Client needs and goals and amend the CMP accordingly and (iv) seek Client feedback about their experiences.
  • Contribute to the development and implementation of annual business/work plans and innovative service delivery
  • Development of networks and ongoing liaison with key stakeholders to meet client needs and ensure caseload growth targets are achieved


Minimum qualifications

Diploma of Community Services (Case Management); or Bachelor of Applied Sciences (Community Services) or Bachelor of Social Work or an equivalent alternate qualification

Essential experience

  • Demonstrated experience of case management supporting vulnerable and disadvantaged people with multiple barriers to social and economic participation
  • Demonstrated experience working with people from culturally diverse backgrounds in a culturally responsive and Aboriginal and Torres Strait islander people
  • Demonstrated understanding of support needs of people experiencing crisis (eg. family violence, child protection, substance abuse, acute mental illness etc);
  • Understanding of the impact of socio-economic disadvantage, trauma and other factors on education and employment
  • Strong background in conducting needs assessment with clients; developing, implementing and reviewing case plans;
  • Demonstrated experience in working with databases, spreadsheets and Microsoft Office applications;
  • Experience delivering person-centred and strengths-based case management
  • Understanding of the various vocational and non-vocational barriers to social and economic participation that will need to be considered when case managing OPP clients
  • Current NSW drivers licence required.
