About the role
We’re seeking a compassionate and experienced Support At Home Team Leader (previously HCP) to lead our care operations. In this leadership role, you will oversee and mentor the SAH Care Partner team, ensuring the seamless delivery of Support at Home services (previously Home Care Package services) that align with each client’s individual needs, preferences, and goals.
As an Team Leader, you will oversee and drive the coordination of community care services for older adults, championing excellence in every aspect of service delivery. You’ll provide leadership in coordinating care, supporting your team to work with clients, families, health professionals, and service providers to manage risks, maintain care plans, and deliver exceptional, high-quality services.
Key responsibilities include:
- Lead, coach, empower and develop a team of 6 Care Partners, fostering confidence, capability and resilience.
- Support and uplift the team in managing complex client needs, including involvement in incidents, complaints and escalations.
- Join client visits when required to guide best practice and ensure quality outcomes.
- Work collaboratively with leadership teams to align processes and uplift practices
- Contribute to service growth, intake efficiency, and client experience improvements.
About you
While demonstrated leadership experience within Support at Home (SAH) and/or Home Care Packages (HCP) is a must, you will also have:
- Demonstrated ability to lead in complex service environments.
- Experience supporting teams through incidents, client complaints and escalation pathways.
- A passion for capability building within your teams.
- Ability to work in a fast-paced, high-growth environment with a positive, growth-mindset approach.
- Some knowledge of intake/business development is a bonus!
