At Sai Home and Community Care, part of the Kinyara Health Group, we’ve designed the Care Partner role to focus on what matters most – supporting older Australians to live well at home.

Our Care Partners are supported by modern systems, streamlined processes and strong operational support, helping reduce administrative pressure so you can focus on building relationships and delivering great client outcomes.

Due to continued growth, we’re looking for a Care Partner to join our team on a permanent full-time basis, based from our Carrum Downs office with hybrid flexibility.

The Role

You’ll support a portfolio of clients receiving Support at Home / Home Care Packages, helping them remain safe, independent and connected in their own homes.

Key responsibilities include:

  • Conducting client assessments and developing personalised care plans
  • Coordinating services with healthcare professionals, support workers and families
  • Monitoring and reviewing care plans as client needs change
  • Supporting clients and families through their Support at Home journey
  • Maintaining accurate documentation and meeting compliance requirements

About You

You may already work in aged care or community services and be looking to step into care management.

We’d love to hear from you if you currently work as a:

  • Service Coordinator
  • Rostering Coordinator
  • Intake Officer
  • Or another community aged care role supporting clients

You will bring:

  • Experience working in the aged care or community care sector
  • Strong communication and relationship-building skills
  • Good organisation and problem-solving ability
  • Confidence using electronic systems
  • A genuine passion for supporting older Australians
