About the Role

We have an exciting opportunity for a Support Coordinator to work for the Safe at Home team for the Family Services department.

Supported by the Team Lead, the Support Coordinator provides client facing recovery interventions and plays an important role in improving the health and social wellbeing, housing and overall quality of life for clients, maximising self management, independence and participation in the community.

This is a Permanent, full time position working 76 hours per fortnight, Monday to Friday.

What we’re looking for

The successful applicants will be required to meet the following criteria:

  • Relevant qualifications in a related discipline at degree, diploma, or certificate level/less formal qualification with work or lived experience to perform sufficiently at this level/or demonstrated work experience at the level.
  • Personal goal planning skills that contribute to the achievement of client goals in line with the Model of Care.
  • Skills in reflective practice and highly developed report writing using analytical reasoning.
  • Ability to support, mentor and work alongside supporting staff members.
  • Advanced computer skills, proficient in Microsoft Office and client reporting system.
  • Ability to engage and maintain professional relationships with clients, and their wider formal and informal supports, with multiple needs and challenges.
  • Demonstrated understanding of complex issues facing people who experience chaotic life circumstances such as homelessness, AOD use, family/domestic violence and mental health issues.
  • Willingness and ability to work within and contribute to the vision, mission, core values and the guiding principles of the organisation.

  • Demonstrate knowledge and understanding of diversity and social inclusion-including gender, sexuality, disability, cultural, age and linguistic diversity and Aboriginal culture.
