About the Role
The Stronger Ground program provides support to individuals living with the challenges associated with severe and persistent mental health challenges. As a Support Coordinator you provide a holistic approach to psychosocial case management alongside other support networks.
As a Support Coordinator you will work in a trauma informed way and liaise with Support Workers and Clinical Case Managers and provide capacity building support as determined by the client’s recovery goals. The focus of work is to support clients to reconnect with community and improve their mental health and wellbeing and overall quality of life. The successful applicant will have experience in working with clients in a family inclusive way, working in a team environment, and liaising with mental health professionals, housing providers and other appropriate services to support the client’s recovery journey.
The Support Coordinator position is at the forefront of the organisation in providing client facing recovery support interventions. Supported by the Team Lead, you will play an important role in improving the health and social wellbeing, and overall quality of life for clients, maximising functional recovery, self-management, independence, and participation in the community.
This is a permanent full time position, working 38 hours per week between Monday to Friday. This position requires travel from our corporate office in Northbridge to areas of the North Metropolitan region and other Ruah sites as required.
What we’re looking for
The successful applicant will be required to possess the following criteria:
- Relevant work experience in a similar position.
- Personal goal planning skills that contribute to the achievement of client goals in line with the Model of Care.
- Skills in reflective practice and highly developed report writing using analytical reasoning.
- Ability to support, mentor and work alongside supporting staff members.
- Advanced computer skills, proficient in Microsoft Office and client reporting system.
- Ability to engage and maintain professional relationships with clients, and their wider formal and informal supports, with multiple needs and challenges.
- Demonstrated understanding of complex issues facing people who experience circumstances such as, AOD use, family/domestic violence and mental health issues.
- Willingness and ability to work within and contribute to the vision, mission, core values and the guiding principles of the organisation.
- Demonstrate knowledge and understanding of diversity and social inclusion- including gender, sexuality, disability, cultural, age, and linguistic diversity and Aboriginal culture.
Relevant qualifications in a related discipline at degree, diploma, or certificate level are desirable, but not essential.
