About the Role

Street to Home provides support to people experiencing homelessness to obtain and maintain long-term accommodation while receiving holistic case management support.

As a Support Coordinator, you will work directly with individuals experiencing homelessness and complex life circumstances, supporting them to secure housing, improve wellbeing and build sustainable connections within their community. Working within Ruah's recovery-oriented Model of Care, you will deliver strengths-based case management, advocacy, service coordination and practical support to help clients achieve their goals and improve their quality of life.

You will proactively engage people who may be disconnected from support services, develop recovery plans, coordinate referrals, advocate for access to housing and community services, and work closely with Support and Peer Workers and external stakeholders to achieve positive outcomes.

This is a full-time max-term position until 30 June 2027.

What We're Looking For

To be successful in this role, you will have:

• Experience providing case management, support coordination or community-based support services.
• Strong ability to build professional, strengths-based relationships with people experiencing complex challenges.
• Demonstrated understanding of homelessness, mental health, alcohol and other drug issues, and family and domestic violence.
• Well-developed report writing, assessment and analytical skills.
• Strong organisational skills with the ability to manage competing priorities.
• Advanced computer skills, including experience using Microsoft Office and client management systems.
• A commitment to recovery-oriented, person-centred and culturally safe practice.
• A genuine commitment to Ruah's vision, mission and values.

Desirable

• Qualification in Community Services, Social Work, Psychology, Human Services or a related discipline, or equivalent experience.
• Experience working within homelessness, housing, mental health or community services programs.
• Understanding of diversity, social inclusion and culturally responsive practice, including working with Aboriginal people and communities.

• Salary aligned with Level 4 of the SCHADS Award, commencing at $92,575 plus superannuation.

• 5 weeks annual leave (FTE) plus 17.5% leave loading.
