Support Worker - Mental Health and Wellness

Support people living with severe mental health challenges to achieve recovery goals, build independence and improve wellbeing. As a Support Worker with the Stronger Ground Project, you'll deliver personalised psychosocial support that creates meaningful change for individuals, families and communities across Mandurah and the Peel region.

About the Role

The Stronger Ground Project provides personalised psychosocial support for people living in the community with severe mental health challenges. Support may include service coordination, goal planning, maintaining accommodation, employment support, community participation and supporting carers.

As a Support Worker, you will work one-on-one with participants to identify goals, coordinate supports and develop practical strategies that promote recovery, independence and social inclusion. You will play a key role in supporting individuals to improve their wellbeing, strengthen community connections and achieve meaningful outcomes.

Reporting to the Team Lead, this role is ideal for someone who is passionate about person-centred support and empowering people to create positive change in their lives.

This is a full-time permanent position based in Mandurah.

Key Responsibilities

• Provide personalised psychosocial support to individuals living with severe mental health challenges
• Support participants to identify goals and develop recovery-focused support plans
• Assist individuals to maintain stable accommodation and strengthen independent living skills
• Support access to employment, education, health and community services
• Coordinate services and referrals to promote positive participant outcomes
• Build strong, respectful and professional relationships with participants, carers and stakeholders
• Deliver strengths-based, recovery-oriented and person-centred support
• Maintain accurate client records, case notes and reporting requirements
• Promote community participation, social inclusion and wellbeing
• Work collaboratively with mental health services, GPs, psychiatrists and community partners
• Participate in supervision, team meetings and ongoing professional development

What we’re looking for

The successful applicant will demonstrate:

• Completion of, or progress towards, a relevant qualification (Certificate II or III), relevant work experience and/or lived experience
• Experience supporting people with mental health challenges and complex needs
• Ability to engage with individuals with diverse needs and build professional relationships
• Ability to work independently and contribute positively to a team environment
• Demonstrated emotional maturity, resilience and ability to model positive behaviours
• Strong interpersonal and communication skills
• Understanding of recovery-oriented, strengths-based and trauma-informed practice
• Basic computer skills including Microsoft Office and client management systems
• Knowledge and understanding of diversity, gender and social inclusion, particularly Aboriginal culture
• Commitment to Ruah's vision, mission and values

The following are considered desirable:

• Experience working in community services or mental health services
• Knowledge of psychosocial support models and recovery-based practice
• Experience supporting people to navigate community services and systems
• Knowledge of local support services and referral pathways

• A salary aligned with Level 2 of the Ruah Enterprise Agreement (and the SCHADS Award), starting at $71,800 + Super.
