Your Opportunity

We are seeking a Physiotherapist to deliver care across both Community and Residential Aged Care settings.

In this hybrid role, you’ll support older Australians to remain active, independent, and connected. Whether that’s in their own homes or within residential facilities. You’ll work as part of a large, collaborative multidisciplinary team, delivering personalised, evidence-based interventions that make a genuine difference.

Using our Plena Care Pathway, a reablement-focused model, you’ll help clients achieve meaningful functional outcomes at every stage of their aged care journey.

In the community setting, you’ll work with clients referred through Home Care Packages (HCP), Short Term Restorative Care (STRC), Commonwealth Home Support Programme (CHSP), as well as private clients.


What You Will Be Doing

You’ll manage a diverse caseload across both settings, including:

  • Mobility and functional assessments
  • Restorative and reablement therapy
  • Falls prevention and risk minimisation
  • Pain management and musculoskeletal care
  • Pressure care and skin integrity management
  • Equipment prescription and home modifications
  • Active ageing and group exercise programs

You’ll deliver both 1:1 therapy and group-based interventions, while collaborating closely with care teams, families, and other clinicians to provide holistic, person-centred care.

What We Are Looking For

  • Bachelor’s degree in Physiotherapy
  • Current AHPRA registration
  • A passion for reablement and improving quality of life for older people
  • Australian working rights
  • Willingness to obtain NDIS Worker Screening and a National Police Check
