The Opportunity
As an Outbound Care Coordinator within the Get Moving Outbound program, you will support Plena Healthcare’s proactive customer care review initiatives. This includes engaging with customers, confirming their interest in services, and coordinating wellness and mobility assessments.
Working from our Norwood office, you’ll join a supportive and collaborative team that values initiative, communication, and customer outcomes.
Key Responsibilities
- Proactively contact customers to determine their interest in participating in a wellness assessment conducted by Plena Healthcare
- Clearly explain services and obtain appropriate customer consent for assessments and follow-up care
- Coordinate and schedule mobility and wellness assessments where appropriate
- Create and maintain well-planned, optimised schedules for clinicians delivering assessment services
- Arrange initial appointments, confirming customer details, funding information, and service requirements
- Coordinate ongoing appointments in line with clinician recommendations and customer/case manager approval
- Respond to general enquiries, cancellations, and rescheduling via shared phone line and inbox
- Support workforce planning by reviewing clinician availability and raising resourcing needs where required
- Process planned and unplanned leave requests while maintaining service delivery coverage
- Contribute to continuous improvement of outbound engagement processes
- Undertake other tasks as required from time to time
About You
You are a proactive and customer-focused professional with strong communication skills and confidence engaging with customers in an outbound environment. You have excellent attention to detail, strong organisational skills, and the ability to manage multiple priorities and deadlines effectively. You thrive in a fast-paced team environment and bring a positive, solutions-focused attitude to your work, with the ability to support quality assurance processes and provide day-to-day team oversight and guidance.
Experience in rostering, scheduling, administration, customer service, outbound contact centres, or healthcare coordination will be highly regarded, while knowledge of Allied Health services will be an added advantage.
