Your Opportunity:
As a Team Leader at Plena Healthcare, you will provide leadership and support across a dedicated region, guiding operational and clinical services while fostering strong, positive relationships with residents, families, and internal stakeholders. You will be part of a collaborative, multidisciplinary team delivering personalised, high-quality care across Residential Aged Care and Community settings.
In this role, you will oversee the delivery of evidence-based mobility and functional programs, care plan reviews, one-to-one therapy, and group-based active aging services, ensuring your team provides efficient, person-centred care that enhances the quality of life for every resident.
Duties will include, but not limited to:
- Provide leadership and guidance to Allied Health teams to ensure high-quality, person-centred care.
- Oversee assessments and clinical recommendations for mobility, transfers, falls prevention, equipment needs, skin integrity, pain, and joint management.
- Ensure best practice in care planning, documentation, and adherence to professional standards.
- Support team development through coaching, feedback, and performance guidance.
- Adapt to the evolving needs of Plena and the aged care facilities.
