About the Role:
Reporting to your Dietetics Team Leader, you’ll deliver high-quality, person-centred dietetic services across Residential Aged Care facilities and the local community. This mobile role offers variety, autonomy, and the opportunity to build lasting relationships with clients and colleagues.
What You'll Be Doing:
- Providing individualised nutrition care to adults in aged care and community settings
- Supporting improved nutrition and hydration systems in aged care homes
- Conducting menu and mealtime assessments
- Facilitating engaging education sessions
- Collaborating with external stakeholders and multidisciplinary teams to ensure best outcomes
- Maintaining accurate and timely clinical documentation
- Contributing positively to our team culture and living out the Plena values
- Embracing continuous learning and evidence-based practice
What you'll need to succeed:
- A Bachelor’s or Masters Degree in Dietetics
- Current DA registration
- This is a mobile role and a willingness to travel is essential
- Willing to obtain NDIS workers screening and National Police Check
- Australian working rights
- Proof of Covid-19 & Current Flu Vaccination
