About The Role

We’re seeking a compassionate and organised SAH Care Partner (Support at Home, formerly HCP) to join our team. In this client-focused role, you’ll coordinate personalised care that supports older people to live safely and independently at home.

Working closely with clients, families and health professionals, you’ll ensure services align with each person’s goals, preferences and changing needs.

Key responsibilities include:

  • Ensure care and services are identified, planned and delivered in line with each client's needs, preferences and goals
  • Identify and address risks to client safety, and document accordingly
  • Follow up and resolve incidents and feedback in a timely and appropriate manner
  • Conduct regular client reviews and home visits to keep support plans current and relevant
  • Coordinate and schedule services with internal and external providers
  • Act as a key point of contact for clients, families, GPs and other health professionals
  • Organise family conferences and multidisciplinary case reviews
  • Keep clients (and/or their representatives) informed of any changes to care plans, agreements or budgets
  • Ensure client risks are managed and documented, including through the high-risk client register


About You

You are a compassionate, relationship-driven person with a passion for supporting older people to live safely and confidently at home. You’re a strong communicator, love solving problems, and are confident managing a busy caseload.

You’ll also bring:

  • Case management experience, ideally in aged care or community care (3+ years' ideally)
  • A sound understanding of Home Care Packages (and SAH) and sector compliance requirements
  • Excellent communication and relationship-building skills
  • Ability to manage competing priorities and adapt to change
  • Technical/digital literacy
  • Clinical qualification (e.g. RN/EN) – desirable
