About The Role

We’re seeking a compassionate and organised SAH Team Lead to lead our Support at Home operations. In this role, you will manage and mentor the SAH Care Partners team, ensuring the seamless delivery of Support at Home services (previously Home Care Package services) that meet each client’s individual needs, preferences, and goals.

You will manage a dedicated caseload of clients, giving you the opportunity to stay connected, deliver meaningful person-centred support, and remain grounded in the day-to-day realities of your team’s work.

You’ll review care plans, monitor budgets, manage risk, and ensure services are compliant and aligned to client goals, while also driving team performance, supporting program growth and contributing to continuous improvement.

With the support of a dedicated SAH Admin Support Officer, you can focus on leadership, quality outcomes, and genuine one-to-one client engagement, rather than routine administration.

Key responsibilities include:

  • Lead, support and mentor the SAH Care Partners team, fostering professional growth, collaboration, and a positive team culture.
  • Manage a small caseload of clients, ensuring services are person-centred, meaningful, and that you remain connected to the day-to-day delivery of care.
  • Oversee care coordination and collaboration with clients, families, health professionals, and external providers (with administrative support provided), maintaining strong relationships and community connections.
  • Contribute to continuous improvement, process enhancement, and program growth, shaping the future of Support at Home services.
  • Participate in on-call duties (1 week in 6; not a 24/7 service, RDO provided)


About You

You’re a strong communicator with a genuine commitment to delivering high-quality care. You thrive in a collaborative environment and are confident managing competing priorities. Your attention to detail and organisational skills ensure clients receive the support they need, when they need it.

Above all, you are passionate about working in aged care and thrive in leadership role.

You’ll also bring:

  • Case management experience, in aged care or community services
  • A sound understanding of Home Care Packages (and SAH) and sector compliance requirements
  • Excellent communication and relationship-building skills
  • Strong time management and organisational abilities
  • Demonstrated leadership or team supervision experience
  • A collaborative mindset with a commitment to fostering a positive team culture
  • Proficient computer skills
  • Clinical registration and experience (e.g. EN, RN) (desirable)
