Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
The Complex Care Service is seeking a motivated and team-focused Registered Nurse or Allied Health Professional to join the team as a Care Coordinator providing intensive, time limited intervention to clients with chronic health conditions and/or complex health needs. The role of the Care Coordinator is to provide comprehensive clinical assessment, client education and support, to identify barriers to self-management, make referrals to services and to coordinate the client's care as part of a multidisciplinary team.
You would be joining a dynamic and supportive team of highly experienced professionals and providing care in client's homes and via telehealth. You will receive comprehensive orientation to the role, as well as clinical supervision and professional development opportunities.
