NHW have an exciting internal opportunity for an experienced and motivated Grade 3 Social Worker to join our dedicated Social Work team in a unique and impactful regional role. The Central Hume Social Worker operates in a roving capacity, enhancing access to Social Work services across the Central Hume region, including Alpine Health, Benalla Health, Beechworth Health, Mansfield District Health, and Yarrawonga Health.
This role plays a critical part in supporting patients who transfer from NHW to these regional services. You will conduct high-level psychosocial assessments, contribute to discharge planning, and coordinate ongoing Social Work interventions post-transfer. In addition, you’ll provide support to existing Social Work teams across the region, helping to build capability and improve service delivery.

As a Grade 3 Social Worker, you will contribute to the growth and excellence of Social Work practice in the region, working in alignment with NHW’s vision, values, and standards of behaviour.
