Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.

The primary purpose of the NUM is to provide operational leadership, governance and strategic management of Community Nursing services to optimise service delivery and align with Northeast Health Wangaratta’s (NHW) vision, operational and strategic goals.

The primary aim of Community Nursing services is to support and provide services within patients homes through home based care. Services support patients to achieve their best possible level of health and function and reduce the time spent in hospital. This role oversees:

  • District Nursing Services (DNS)
  • Hospital in the Home (HITH)
  • Palliative Care
  • Community Interlink 

The role requires demonstrated personal attributes that foster collaboration between all departments at NHW and external services, as well as vision
