Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
Northeast Health Wangaratta is seeking a Palliative Care Clinical Coordinator to join the Community Palliative Care Team within the Care at Home division. This multidisciplinary program delivers high-quality palliative care across the Rural City of Wangaratta and Alpine Shire, working closely with Nursing, Occupational Therapy and Psychosocial Support services, with onsite and telehealth support provided through the East Hume Regional Consultancy Palliative Care Service.
The Palliative Care Clinical Coordinator provides a single point of access and coordination for palliative care clients and their carers, supporting smooth and timely care across community, hospital and other care settings of the client’s choice. The role focuses on developing and implementing effective systems and processes for intake, triage, assessment and coordination, while playing a key liaison role between clients, carers, health professionals, care providers and other stakeholders.
Central to this position is ensuring a consistent, person-centred approach to assessment, care planning and end-of-life care. The role supports the delivery of holistic palliative care aligned with the National Palliative Care Standards 2024 and the Victorian Strengthening Palliative Care: Policy and Strategic Directions 2011–2015, ensuring compassionate, coordinated and high-quality care for clients and their families.
