NHW have an exciting internal opportunity for a passionate and experienced staff member to join our team as a Sub-acute Ambulatory Care (SACS) Co-ordinator.

In this vital role, you will provide day-to-day operational leadership and coordination across the SACS and Transition Care Program (TCP) services. The role is key to delivering high-quality, goal-directed care that supports clients' independence, promotes hospital admission avoidance, and ensures smooth patient flow throughout Northeast Health Wangaratta (NHW).

As SACS Co-ordinator, you will:

  • Support and guide staff in delivering holistic, person-centred care.
  • Work collaboratively across NHW departments and with external services.
  • Promote NHW as a leading regional health care provider.
  • Lead with compassion, initiative, and a strong team-based approach.
