Salary/ Hourly rate:  Grade 2: $44.7711 - $51.4632 plus 11% superannuation

 

About the Role:

NCN Health have an exciting opportunity for a Social Worker to join our Allied Health and Chronic Disease team at NCN Health.

The Social Worker works as part of the multidisciplinary Community Health and Wellbeing team at NCN Health, providing professional social work services and programs for a diverse range individuals, carers and families across the organisation including our acute unit, Community Health department, Moira Community Rehabilitation Centre Residential Aged Care facilities, the Moira Palliative Care Service and the Moira Mental Health Service.

What we are looking for:

  • Vaccination against Covid-19 is a mandatory requirement.
  • Bachelor of Social Work or equivalent tertiary qualification, providing eligibility for membership of an accredited and relevant professional association EG: Australia Association of Social Workers.
  • Minimum two years post graduate experience working as a Social Worker in a clinical setting.
  • Demonstrated experience in the provision of a range of evidenced based social work interventions, including psycho-social assessment, counselling, social casework, family work, group work and community liaison supports.
  • Ability to establish, maintain and adapt effective therapeutic partnerships with individuals, their care givers and family in a range of clinical areas to support emotional, social and mental health wellbeing.
  • Demonstrated experience in providing Social Work services within a client focused approach to care delivery that promotes client independence and self-management.
  • Excellent interpersonal, written and communication skills, with an ability to produce quality reports.
  • Well established organisational and time management skills with the ability to prioritise workload.
  • Ability to work collaboratively as an individual practitioner within a multidisciplinary team environment, without direct supervision.
  • Strong computer literacy skills, including the ability to utilise computer software programs and Microsoft Office products.
