Position: Intake Clinician
Location: Across NCN Health
EFT/Hours: Up to 1.3 EFT available
Status: Permanent Part Time
Hourly Rate: (dependent upon Enterprise Agreement) plus 12% superannuation
Enrolled Nurses: IB66-IB68 (Cert IV Year 1-2) & IB69 to IB72 (Diploma Level 1 Year 1-5)
Registered Nurses: YP2 - YP9 (RN Grade 2 Year 1-8)
Allied Health Professionals: Grade 1 - dependant on relevant qualifications and experience
About the Role
NCN Health is seeking an Intake Clinician to join our Allied Health and Chronic Disease Team.
The Intake Clinician provides a welcoming single point of entry to Allied Health and Chronic Disease services at NCN Health. The position facilities streamlined and timely access for clients to a wide range of community-based health services and programs across the Integrated Community Health department.
The Intake Clinician works as part of a multidisciplinary health care team and will effectively engage, screen, triage, assess, refer, and coordinate service access for clients that contact or are referred to the diverse range of community health and wellbeing services, including all nursing, allied health, social support and mental health programs.
The Intake Clinician will also provide support and education to the community in relation to service access, service information and referral pathways as required.
About You
- Vaccination against seasonal Influenza (mandatory)
- Vaccination against Covid-19 (recommended)
- Appropriately qualified in health-related discipline, with current registration with relevant regulation agency eg. Australian Health Practitioner Regulation Agency (AHPRA) as appropriate
- Demonstrated skills and experience to undertake intake, screening and assessment activities, to identify physical, mental, cognitive and behavioural conditions, issues and risks of harm, to determine client’s needs, and to assist clients to access appropriate services.
