Position:       Project Lead Sepsis Collaborative (SCV project)

Location:      NCN Health

EFT/Hours:  0.2EFT (16 Hours Per Fortnight)

Status:          Fixed Term until 31/5/2026

Salary/ Hourly rate: To be determined based on successful applicants' current classification

 

About the Role:

NCN Health is seeking a dynamic and motivated individual to take on the Project Lead role in the Sepsis Collaborative Project with SCV.

 The Project Lead will be responsible for the following:

•            Serve as the central point of contact for sepsis care initiatives within NCN Health

•            Coordinate project timelines, deliverables, and engagement activities across NCN Health

•            Oversee the implementation and embedding of sepsis clinical pathways

•            Ensure alignment with the ACSQHC Sepsis Clinical Care Standard

•            Identify and address site-specific barriers to pathway adoption

•            Support early identification, timely treatment, and appropriate escalation of sepsis cases

•            Lead audits, data collection, and quality improvement initiatives related to sepsis care- Report as per SCV KPI requirements

•            Facilitate staff training and engagement

•            Represent NCN Health in SCV collaborative activities and data sharing forums

•            Integration of sepsis protocols into clinical workflows

•            Regular reporting to the STP coordinator and clinical governance committee

•            Engagement with SCV collaborative forums and improvement cycles

•            Promote awareness of the sepsis initiative at all levels of NCN Health

•            Use data to identify trends, risks, and opportunities for improvement

•            Foster collaboration across with other Hume Region Health services and active participation in the CoP

·        Strong analytical and problem-solving skills

·        Excellent communication and interpersonal skills
