EFT/Hours:  0.2 EFT (16 Hours Per Fortnight)

Status:         Fixed Term - Part Time (Until June 2028)

Hourly Rate: Dependent on qualifications and experience plus 12% superannuation 

About the Role

NCN Health is seeking an enthusiastic Carer Support Coordinator to join our team in Numurkah. Funded under the Department of Families, Fairness and Housing's Support for Carers Program, this rewarding role is a fantastic opportunity to make a meaningful impact in our community.

The Carer Support Coordinator is responsible for the provision of information, education and resources, and supports the planning and coordination of flexible respite services to support local carers who provide unpaid care to a family member, friend or person who is frail aged, has dementia, a disability, mental illness or a medical condition.

The Carer Support Coordinator will assess the needs of the individual carers and work with them to plan and coordinate the provision of supports and services that meet their individual circumstances.

The NCN Health Carer Support Program provides person centred care and support via provision of flexible respite and support services for unpaid carers of people with care needs. Services aim to assist in meeting the diverse and individual needs of carers, promoting improved health and wellbeing of carers, and those for whom they care.

The Carer Support Coordinator is an integral part of the multidisciplinary Community Aged Care team.

About You

  • Vaccination against seasonal Influenza (mandatory)
  • Vaccination against Covid-19 (recommended)
  • Tertiary qualification in a health related discipline, including Nursing, Disability, Welfare and/or Community Services practice including current registration with relevant regulation agency eg. Australia health Practitioner Regulation Agency (AHPRA) if/as appropriate.
  • Demonstrated experience and well developed skills in the delivery of assessment, care planning and coordination of services for individuals and families with diverse needs.
  • Understanding of the role, needs and concerns of carers and a commitment to their rights and entitlements
  • Ability to work without direct supervision, whilst remaining an integral part of a multidisciplinary health team, including the ability to initiate and maintain effective professional relationships.
  • Excellent interpersonal, communication and organisational skills with ability to develop and maintain collaborative relationships with internal and external stakeholders.
  • Excellent time management and organisational skills with an ability to efficiently manage multiple demands and tasks simultaneously.
  • Competent in using Microsoft products (Outlook, Word, Excel) and well-developed computer skills and experience including accurate and timely client data management.
