Position: Care Finder Project Officer
Location: Across NCN Health
EFT/Hours: 0.6 EFT to 1.0 EFT & Casual role available
Status: Fixed Term Part Time to Full Time (until July 2026) /
Casual July 2027
Hourly Rate: $40.1125 - $50.7105 per hour plus 12% superannuation plus casual loading if applicable
About the Role
NCN Health have an exciting opportunity for a Care Finder Project Officer to join our team at NCN Health.
The Care Finder program aims to connect and engage older people who have significant difficulty accessing aged care services and are at risk of "falling through the cracks."
The position will be responsible for supporting and providing intensive assistance senior members of the community to understand, access aged care services and connect with relevant support in the community.
About You
- Vaccination against seasonal Influenza (mandatory)
- Vaccination against Covid-19 (recommended)
- Experience in the community aged care sector with demonstrated experience in assessment, care coordination / case management or advocacy or other relevant role within a service coordination context
- Relevant qualification (e.g: Social Work, Human Services, Aged Care, Community Services or Health)
- A demonstrated understanding of diversity issues as they relate to individuals and their engagement with community care services.
- Excellent interpersonal skills
- Familiarity with the service coordination framework including the key elements of initial contact, initial needs identification, assessment, care planning and the service coordination tool templates and standards
- Highly developed verbal and written communication skills with a demonstrated ability to write in a way that is clear, appropriate and readily understood by the target audience
