Position:         Case Manager - Home Care Packages

Location:        Cobram

EFT/Hours:  0.2 EFT (16 Hours Per Fortnight)

Status         Part Time Fixed Term to 30 September 2024

Salary/ Hourly rate:   Part Time - $31.1763 - $45.7579 (dependent on qualifications and experience)

 About the Role:

NCN Health have an exciting opportunity for a Case Manager to join our Community Aged Care and Disability team in Cobram. The Case Manager is responsible for delivering person centred case management whilst working as part of a multidisciplinary team in the provision of a client focused case management service to support clients in the NCN Health Home Care Package program to remain safely and as independently as possible in their own homes. The Case Manager will work closely with clients, General Practitioners and other community-based aged care services to develop the partnerships necessary to deliver a high-quality service.

What we are looking for:

  • Vaccination against Covid-19 is a mandatory requirement
  • Demonstrated experience in the provision of case management services
  • A relevant post-secondary qualification and experience relating to an area of health and / or community services practice
  • A demonstrated understanding of diversity issues as they relate to individuals and their engagement with community care services.
