Position: Clinical Lead –Murray PHN Chronic Disease Exercise and Education Program
Location: Across NCN Health
EFT/Hours: 0.6 EFT (48 Hours Per Fortnight)
Status: Part Time Fixed Term until June 30, 2027
Hourly Rate: Dependent upon qualification and skill level and 12% Superannuation
About the Role
NCN Health has an exciting opportunity for a Clinical Lead (Allied Health or Nursing) to join our Allied Health and Chronic Disease team.
The Clinical Lead will work under the direction of the Clinical Coordinator and is responsible for the implementation and facilitation of the newly funded Murray PHN Chronic Disease Exercise and Education Program. This program is funded for 12 months and is in partnership with Yarrawonga Health. The program will deliver direct clinical services through a structured exercise and education group program for people living with chronic conditions - cardiovascular (CVD), chronic obstructive pulmonary disease (COPD), and diabetes who reside within the Moira Shire. The program will be delivered at each of the 3 campuses both in-person, virtually and will also include some outreach locations.
The role will coordinate and facilitate programs at each of the 3 campus locations as well as a virtual program, home based programs and facilitate sessions at outreach locations. The role will promote health and wellbeing to individuals and groups aimed to improve, maintain and promote the physical health and wellbeing with a strong emphasis on client self-management skills within a social model of health framework.
About You
- Vaccination against seasonal Influenza (mandatory)
- Vaccination against Covid-19 (recommended)
- Bachelor degree of Physiotherapy, Exercise Physiology or Nursing and current registration with Australian Health Practitioner Regulatory Agency (AHPRA)
- Demonstrated high level experience in the design, delivery and evaluation of exercise, lifestyle and behaviour modification programs for the prevention and management of chronic disease conditions in both individual and group settings.
- Demonstrated experience in providing services with a client focused approach to care delivery that promotes client independence and self-management.
- Excellent interpersonal, written and communication skills, with an ability to produce quality reports.
- Well established organisational and time management skills with the ability to prioritise workload.
- Ability to work collaboratively as an individual practitioner within a multidisciplinary team environment, without direct supervision.
- Strong computer literacy skills, including the ability to utilise computer software programs and Microsoft Office products
