Clinical Care Coordinator- District Nursing

Permanent Full-Time - 80 hours per fortnight

We are seeking a dedicated Clinical Care Coordinator to join our Community/District Nursing team and play a pivotal role in delivering exceptional patient care and service coordination. This position offers an opportunity for career progression. Work in a dynamic team environment where you will be responsible for managing complex patient pathways, providing clinical guidance, and ensuring patient care is delivered in accordance with organisational policy and procedure, NSQHS, and the Strengthened Aged Care Quality Standards

Key Responsibilities

The Clinical Coordinator will be responsible for:

  • Managing patient referrals, admissions and daily operation of the service
  • Managing IPM and collaboration with internal departments within the organisation in relation to patient care
  • Managing My Aged Care (MAC) portal and admissions/discharges and ensuring existing patients > 65 years are linked with MAC
  • Managing Post Acute Care referrals and discharges
  • Liaising with HCP providers
  • Managing NDIS service agreements and fund trackers
  • Serving as central contact for external and internal services, hospitals and medical professionals
  • Developing teamwork through role modelling and effective communication processes
  • Providing clinical care to patients
  • Ensuring delivery of patient quality care in accordance with the organisation's policy and procedures, NSQHS standards, Aged Care Quality Standards (1st November 2025)
  • Managing daily allocation lists
  • Overseeing clinical assessments and documentation
  • Accepting accountability and responsibility for their own actions within their scope of practice
