Social Worker – Primary Care Centre

Location: Mansfield | Status: Fixed Term – 12 months Parental Leave Cover (0.4 EFT / 2 days per week)

1st September 2025 - 7th September 2026

🩺 About the role

We are seeking an experienced and compassionate Social Worker to join us at Mansfield District Hospital on a fixed term basis (parental leave cover at 0.4 EFT). The Social Worker works as part of a Primary Care Centre team and is accountable for provision of a range of Counselling services for Community Health Clients, Inpatients, Aged Care residents and community education programs. Working as part of a multi- disciplinary team and in accordance with the objectives of Mansfield District Hospital, this position is required to coordinate, deliver and evaluate effective and accessible services within Mansfield District Welfare and other contracted service sites.

Your key responsibilities will include but not limited to:

  • Deliver clinical social work services to clients, families, and community groups.
  • Provide care and counselling across inpatient, outpatient, aged care, and community programs.
  • Promote collaborative, person-centered care to support positive health outcomes.
  • Complete assessments and maintain accurate clinical records.
  • Contribute to service planning, budgeting, and meeting strategic goals.
  • Develop and manage work plans to ensure effective service delivery.
  • Participate in ongoing professional development.
  • Provide leadership aligned with organisational values.
  • Manage Emergency Relief services.
  • Develop and run support groups in aged care and the community.

📝 Key Selection Criteria

Essential

  • Tertiary qualification in Social Work and eligibility for AASW registration.
  • Experience in hospital, community, or primary health care settings.
  • Ability to manage complex clients and work effectively in a multidisciplinary team.
  • Strong communication, time management, and organisational skills.
  • Proficient in Microsoft Office (Word, Excel, Outlook).
  • Commitment to confidentiality, professional ethics, and MDH values including Great Care.
  • Current Working with Children Check and evidence of required flu vaccination.

Desirable

  • Experience in aged, acute, or primary care settings.
  • COVID-19 vaccination (highly recommended per ATAGI guidelines).
