About the role

The Case Manager is responsible for the provision of case management and support services to young people living in residential homes. The Case Manager will participate in team planning with residential care workers, other case managers, other staff and volunteers. Furthermore, they are responsible for contributing to best practice Occupational Health & Safety, and positively contributing to the development of MacKillop Family Services.

The Case Manager will Develop, monitor and regularly review each placement against the Looking After Children (LAC) Care Plan and DHHS Case Plan, Engage young people in goal setting, Participate in relevant formal and informal Case Plan meetings, Care Team Meetings, professionals meetings and other forums pertaining to the planning and care of the young people residing in the residential units ensuring that the statutory requirements of case management for all cases are met, Provide individual support and guidance to young people experiencing difficulties in relation to education, vocational training, employment, health, family and undertake appropriate case notes and service recording on CRIS/CRISSP or as required. 


This is a Permanent Full-time opportunity based in Footscray, VIC within our Residential Care Program. The position reports to the Coordinator Residential & LT Case Management.

 

Key Selection Criteria:

To be successful in this role, you will have:

  • Appropriate tertiary qualifications.
  • Sound professional knowledge and theory base.
  • Extensive experience in adolescent welfare practice.
  • Demonstrated professional case management skills, especially in crisis intervention and behaviour management.

You must have a:

  • Valid and current Driver’s Licence;
  • Satisfactory Criminal History Check (paid for by MacKillop);
  • Valid Employee Working with Children Check;
