The AN-ACC & Clinical Systems Coordinator will operate across multiple sites and will report directly to the General Manager – Clinical Systems. This role is responsible for monitoring, analysing, and reporting AN-ACC and clinical outcomes to relevant stakeholders across the organisation.

Key Responsibilities:

  • Continuously monitoring and reviewing AN-ACC with Management and Clinical teams across nine aged care homes to ensure residents’ classifications accurately reflect care burden within the home.
  • Ensuring AN-ACC assessments have been appropriately scheduled for all new admissions and residents with changed care needs and monitoring the outcomes of these assessments to ensure they reflect the predicted outcomes.
  • Providing guidance, support and education to Clinical teams with regard to documentation requirements to ensure AN-ACC assessment outcomes are accurate.
  • Working with Home Managers and Clinical teams to review and monitor clinical outcomes and provide oversight and education with clinical documentation.
  • Assisting with administration and improvement of clinical systems and processes.
  • Providing support to Clinical Care Coordinators/Care Managers to promote excellence in care delivery, enhance clinical outcomes, and uphold the highest standards of resident care and compliance.
  • In collaboration with Clinical Care Coordinators/Care Managers, collating and submitting data for NQIP reporting on a Quarterly basis.

Essential Skills and Experience:

  • Registered Nurse (RN) or Enrolled Nurse (EEN) with a current AHPRA Registration
  • Knowledge of Australian National Aged Care Classification (AN-ACC)
  • Clinical experience in an aged care setting
  • High level of interpersonal, verbal and written communication skills.
  • High level of statistical analysis with relevant computer skills
  • Experience in working with diverse clinical teams in residential aged care
  • Strong understanding of Aged Care Quality Standards and regulatory frameworks
  • Strong organisational and time management skills, with the ability to manage multiple priorities
  • Passion for aged care and improving the quality of life for residents

  • High level of professionalism and ethical standards
  • Ability to work independently and as part of a team
