Overview

Kyabram District Health Service (KDHS) is a medium-sized rural health service that delivers a range of programs and services including acute, subacute, residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram, Stanhope and Tongala.

KDHS is a passionate inclusive workplace that promotes and values diversity. We are a culturally diverse organisation with a wide variety of people from different races, cultures, age, gender, ethnicity, physical ability, sexual orientation, religious belief, work experience and education background. We believe diversity drives innovation and we will continue to build and nurture a culture where inclusiveness is a reflex and where there is a deep sense of pride, passion and belonging that transcends any role.

About the Role

Following the successful establishment of the After Hours Primary Care Service through Murray Primary Health Network funding, this role has transitioned from a project-focused position to an operational leadership role responsible for the ongoing coordination and delivery of the service.

The After Hours Primary Care Services Coordinator plays a pivotal role in ensuring Campaspe residents have timely access to urgent, non-life-threatening primary care services outside standard business hours. The role supports a sustainable model of care that strengthens local health system integration, improves patient access, and helps reduce avoidable presentations to Urgent Care Centres and Emergency Departments.

Reporting to the Director of Primary Health & Community Services, the Coordinator is responsible for the day-to-day management and operational oversight of the service, including workforce coordination, service planning, clinical governance support, performance monitoring, and stakeholder engagement. A key responsibility is the coordination of the multidisciplinary after-hours workforce, including General Practitioner and nursing rostering, to ensure safe, efficient, and responsive service delivery.

The position works closely with medical, nursing, administrative and executive staff, as well as external healthcare providers, to continuously improve service effectiveness, patient experience, and health outcomes for the Campaspe community.

Key Selection Criteria

Essential:

·        Bachelor of Nursing, Allied Health or Health Administration qualification (or equivalent) with and relevant clinical experience

·        Proven experience in a clinic coordination or healthcare administration role with a strong understanding of GP clinic operations, patient flow, appointment systems and familiarity with Medicare billing, private patient billing for medicate ineligible patients and health fund processes

·        Exceptional interpersonal skills with a track record of successfully engaging diverse stakeholders, in particular GPs, primary health care workers and staff in UCC/EDs. Ability to build and maintain strong relationships, fostering a positive and collaborative environment.

·        Excellent verbal and written communication, and problem-solving skills. Skilled in handling difficult conversations and complaints professionally. Ability to monitor clinic performance metrics, convey complex information clearly and concisely. Experience in preparing relevant stakeholder communications, project-specific reports and presentations for various audiences including KDHS Board and MPHN.

·        Demonstrated ability to work autonomously, adapt to changing circumstances and to adjust strategies accordingly. Ability to identify operational issues and implement effective solutions, including the ability to manage unexpected situations (e.g. staff shortages, patient surges). Willingness to embrace innovation and explore new solutions to overcome an identified community need.

·        Understanding of privacy legislation and healthcare accreditation standards.

·        Familiarity with primary healthcare operations, policies, and regulatory requirements.

·        Experience in medical workforce, healthcare or practice management.

·        Experience in undertaking accreditation application and assessment in a GP Practice or Healthcare setting.

Desirable:

·        Postgraduate qualifications or experience relevant to the Clinical Coordinator role or related studies.
