Case Manager/Support Coordinator

 Fixed Term Part Time| 0.8 EFT

Community Interlink is a Home Care Package management agency and NDIS Support Coordination provider throughout the Hume Region. Community Interlink is a consortium model comprised of 18 public health services. Goulburn Valley Health leads a team of specialized employees across the region providing supports within their own localities to regional consumers. Community Interlink has outlets in Benalla, Broadford, Cobram, Shepparton, Seymour, Numurkah, Wangaratta, Wodonga and now Kyabram.

About the Role

KDHS are seeking an enthusiastic Case Manager to establish a presence in the local community, to support package recipients to build capacity and maintain independence for as long as possible. You will grow a caseload in Home Care Packages area initially, working under the Community Interlink model of service provision. Based in Kyabram with some travel to Shepparton to attend orientation, training and meetings.

We are looking for

  • Commitment to KDHS Values Community, Empathy and Wellbeing
  • The ability to promote and market the service in the local area
  • Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
  • Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of community members, service providers and agencies.
  • A sensitive approach to working with individuals to help them maintain independence
  • Knowledge of the Case Management approach to service delivery in a rural community setting
  • Ability to work as part of a multi-disciplinary team
