About the Role

The Care Coordinator plays a key role in the Partnerships in Positive Ageing (PIPA) program, supporting frail older adults in the Southern Campaspe region to stay active, connected and independent. Acting as the primary contact for participants, you will conduct assessments, identify goals and develop individualised action plans using a person-centred, strengths-based approach.

The role coordinates multidisciplinary services, facilitates referrals, and works closely with GPs, allied health and community organisations to ensure seamless service navigation. You will monitor progress, address barriers such as access or transport, and support safe transitions when participants exit the program. The role also requires accurate documentation, meeting reporting requirements, and contributing to quality improvement, program planning and ongoing service development.

About You

You are a qualified Allied Health (Grade 3) or Nursing professional with strong clinical skills and experience supporting older adults or people with chronic, complex needs. You excel in person-centred care, undertaking assessments, identifying risks and developing effective care plans. You build positive relationships with clients, families and providers, and communicate clearly across multidisciplinary teams. You are organised, proactive and adaptable, able to coordinate multiple services and navigate health and community systems. You maintain accurate records, meet reporting and compliance requirements, and understand quality, safety and accreditation standards.

Experience in care coordination, motivational interviewing, My Aged Care or community seniors programs is highly valued. Above all, you are compassionate, collaborative and committed to helping older people live well in their community.
