The Community Services Manager is responsible for the leadership, coordination, and continuous improvement of the Community Services team at Kyabram District Health Service (KDHS). This team delivers a range of programs and services, including:

  • District Nursing (clinical leadership would move to Acute NUM if successful candidate is not a nurse)
  • Hospital in the Home (clinical leadership would move to Acute NUM if successful candidate is not a nurse)
  • KyConnex - Social Support Program
  • Ky Care
  • NDIS Community Access
  • Home Care Packages and Support Coordination
  • Transition Care Program
  • Community-based outreach initiatives in Tongala and Stanhope, covering Community Awareness initiatives.

This role ensures the effective coordination and delivery of high-quality, person-centred clinical care, particularly in managing complex health conditions in the home and community setting. The manager also supports consumers and their families by facilitating connections across relevant KDHS services.

The Community Services Manager / Community Services Nurse Unit Manager leads clinical operations and is instrumental in driving strategic business growth within the Community Services division. A key focus of the role is ensuring care is aligned with the Australian Commission on Safety and Quality in Health Care (ACSQHC) Standards and KDHS policies—empowering consumers to remain independent in their homes for as long as possible.

This position promotes consumer choice through the development and implementation of flexible, high-quality care services tailored to individual needs, goals, and preferences.

Working collaboratively across the broader KDHS organisation, the role fosters integrated, multidisciplinary support across acute, residential, and community settings. Success in the role requires strong leadership in clinical and operational performance, business acumen, data-driven evaluation strategies, and the ability to assess and monitor outcomes against key performance indicators and regulatory standards.

About the Role

The Nurse Unit Manager supports and coordinates the operational development and enhancement of the Community Services team of Kyabram District Health Service. The Community Service team operates across:

  • District Nursing
  •  Hospital in the Home,
  •  KyConnex - Social Support Program,
  •  Ky Home Care, that includes providing home and personal care services to the local community
  •  Home Care Packages in partnership with Community Interlink and
  • Transition Care Program, in partnership with Bendigo Health
  •  Projects to enhance connection between Acute and home and community programs

 The Community Services Nurse Unit Manager is responsible for ensuring the coordination and delivery of comprehensive person centred clinical care and the management of complex health conditions in the home and community setting. The role will provide elements of support to consumers and their families; as well as links to the KDHS District Nursing team, Hospital in the Home, Social Support Program, Home Care, Transition Care and NDIS Case Management programs.

About You

  •  Current Registration with Nursing and Midwifery Board of Australia as a Registered Nurse or Allied Health Professional with AHPRA or equivalent
  • Maintains composure in high-pressure situations while taking thoughtful, proactive steps to support the community and colleagues
  • Experience leading and/or managing community or aged care services - desirable 
  •  Demonstrated relevant experience within Community Services or a similar health related service - desirable
  •  Demonstrated effective communication through use of information technology, written and verbal means with the ability to utilise a range of communication strategies to negotiate resolutions and manage conflict.
  • Previous experience in assessing and supervising clinical competence and performance of nursing staff/Allied Health and other support staff and the implementation of continuous quality improvement programs, operational policies and procedures and implementation of risk management processes
  • Demonstrated understanding of clinical governance and relevant accreditation standards including The National Safety Quality Health Services Standards, (NQSHS) National Disability Insurance Scheme (NDIS) and Community Aged Care Standards.
  • Previous experience managing program budgets, project management and people management
