About the Role:

- Support and coordinate operational development

- Comprehensive person-centered clinical care

- Management of complex health conditions in the home and community setting

- Link consumers and families to various programs

About You:

- Registered Nurse with Nursing and Midwifery Board of Australia

- Experience in leading and managing community or aged care services

- Strong communication and coordination skills

- Knowledge of clinical governance and accreditation standards
